If you’ve made up your mind and would like me to treat your dog, complete a doggy details form and I will contact you to talk about your dog and confirm a provisional appointment.
If you live in Cornwall or West Devon, I come to you to treat your dog in the comfort of their home. If you live further afield do not despair! I offer distance support options, carrying out assessments and providing support using photos, videos and live chats with you and your dog. See the Services and Prices page for more details and if you’d like to book a distance support package, complete a doggy details form and your starter pack will be with you within a week!

Doggy Details Form
Whether you want to start a Distance Support Programme or arrange our first face to face appointment, your next step is to complete a Doggy Details form.
If there is more than one dog joining us, please complete a form for each of your furry friends.
Vet Consent Form
Please contact your vet and let them know that you would like your dog to have Galen Myotherapy treatment. Download this form and e-mail it to your vet so they can complete and return it to me.
I am happy to contact your vet directly to discuss the treatment and to help you complete the form.
If you have any questions you can call me, or send me an email.
If you are worried about your dog, it is important to get your questions answered as soon as possible. Always talk to your vet about emergencies, or if you think your dog has a new illness or their condition has changed.
I am here 7 days a week for general help and advice relating to Galen Myotherapy and will return your call or e-mail within 24hrs.
If you call and don’t get through I am probably carrying out a consultation, so please leave a message and I will call you back.
